Henry Mayo's Case Management and Social Workers Honor Community Partners

VALENCIA, CA (August 11, 2008) — Henry Mayo's case management staff sometimes face challenges in caring for our patients and ensuring the patient's needs are met when they return home. Following hospitalization, many patients continue to need health care treatments, medical equipment, and personal care. It is important that case managers identify and intervene with high-risk patients to prevent unplanned readmissions.
    Vivian Rayburn, RN, director of case management, reports that Henry Mayo's community partners are critical to their success in providing support and the best care possible to the patients they serve. In appreciation and recognition of their support and contributions, the Case Management department hosted luncheons on June 3rd and July 30th to honor their community healthcare partners.
    More than 50 home health, medical equipment suppliers, companion caregivers, and hospice providers attended the luncheon. Each community partner was recognized and presented with an appreciation certificate and engraved placque.
    "We are proud to have these organizations as our partners," stated Vivian Rayburn, RN, director of case management. "Together, we ensure our patients have the best available resources to receive the best care possible."
    Hospital leadership also attended the celebration, including John Schleif, senior vice president and chief operating officer, and Mark Sender, MD, chief of medical staff.
    "Caring for people takes a special quality and unique ability that not everyone possesses," said Dr Sender. "We thank each of you for the compassionate care you provide daily to our patients and our community."
